Female History:
 |
To record family and medical history of the patient, which will include their sexual, health, surgical and gynecological history, any PAP smear and obstetric summary, any allergies or risk factors, and a chronology of family history, pregnancy events, and previous ART attempts. | |

Physical and Gynecologic Exam:
|
Descriptions of physical and gynecologic exams with direct connection to the ultrasound machine. | |
|
Pelvic Exam:
|
To describe the uterus and ovaries with snapshots and video capture. | |
|
Post Coital Test:
|
To evaluate the interaction between the cervical mucus and sperm. | |
|
OI/IUI Treatment:
|
To schedule the cycle, to follow up with medications and to record ultrasound measurements of ovarian volume, follicular count and endometrial thickness. | |
|
IUI Sperm Prep:
|
To record sperm sample details, motility records before and after wash and insemination data. | |
|
HSG/SHG:
|
To record uterine exploration findings with images. | |
|
Laparoscopy:
|
To examine the pelvic region with a choice to import a template and the possibility to capture snapshots and video streams. | |
Hysteroscopy:
|
To record all uterine findings and capture snapshots and video streams as well. | |
